On a players profile page, in the box with their user profile, under PvP wins there is an option to add rival. It only shows up if they are in the same league as you I think.

It's just gives you a little point tally at the bottom of your explore page.

And so far I've seen, you can make however many people you want as rivals, there is no downside. Just go to gathering >alphas and search up your league leadervoard and just start adding people as rivals

For the Wolves of Warfare event, the game guide says you get an extra claw for defeating rivals. There is an option to add rival on a player's profile page.
