If it's a normal familiar, then once the health is gone, so are they. You can get the elixir to heal them before they go though.

Event familiars (FMC, Biddoo and monthly events) only flee like a wolf does. They won't die until old age takes them ^^

I'd like a couple of regular pack members for Blood Moon and Lunar Falls as well, but it isn't required.
